UnitedHealthcare HMO & HMO-POS Plan Referral Requirements

Effective January 1, 2026

UnitedHealthcare Medicare Advantage HMO & HMO-POS plans will require referrals for specialist visits.

How to Get a Referral

Contact your Primary Care Provider (PCP) before scheduling a specialist appointment.

A referral is not required for:

Audiologists
Optometrists
Ophthalmologists

A referral is required for:

Otolaryngologists (ENT)
Otolaryngologists (Allergy)

Referral Details:

Covers up to 6 visits
Valid for 6 months

Avoid delays in care — check with your PCP today if you need a referral to see a CEENTA doctor!
