Charlotte Eye Ear Nose and Throat Associates will file an insurance claim for most services rendered by our providers as long as we have a current copy of your insurance identification card.  Please help us by bringing your insurance card to each visit.  We accept cash, checks and all major credit cards for copayments, co-insurance, deductibles and any non-covered services.  Copayments are a requirement by your insurance plan and are to be paid at check-out following each visit; if your copayment is not paid, then there will be a service charge added to your visit for billing of your copayment.
