Charlotte Eye Ear Nose and Throat Associates will file an insurance claim for most services rendered by our providers as long as we have a current copy of your insurance identification card.  Please help us by bringing your insurance.   Please help us by bringing your insurance card to each visit.  We accept cash, checks and all major credit cards for copayments, co-insurance, deductibles and any non-covered services.  Copayments are a requirement by your insurance plan and are to be paid at check-out following each visit; if your copayment is not paid, then there will be a service charge added to your visit for billing of your copayment.
